Medical Records Requests

If you are a law firm or records retrieval company, please submit your request through www.chartswap.com. ChartSwap serves as our official custodian of records for these requests, and our office will not process them directly.

For all other medical records requests, the only acceptable method is fax to 337-485-5508. We are unable to process requests submitted by email or phone.
